Question: How do I report a suspected adverse reaction or effect of a medication given to one of my patients?
Answer:
It is a relatively easy process. It involves filling out a 1932a Form for animal drugs, devices, and foods. This form is a one page short form to fill out, and if it is mailed from within the United States there is no postage necessary to send it to the Department of Health and Human Services.
